*Physical forms of past campers are kept on file and are good for three years from the date of the physical.  This form is a generic form and may be replaced by one from the camper's pediatrician.

* *Due to our proximity to the Millstone nuclear power plant, the state of CT requires each camper to complete this form granting or denying permission for camp officials to administer an Iodine Tablet to your child in the extremely rare case of a nuclear disaster.

*** If a camper is taking any mediation (even over the counter allergy/cold medication or ibuprofen such as Advil or Aleve), especially including prescription medication, this form MUST be completed in order for the medication to be present at camp.
