WAIVER FORM
You only need to fill out one waiver form even if you are attending multiple sessions!
If you are attending the CC Preseason Clinic, Conditioning Camp, a Volley Tots session or a Team Clinic this is the only form you need to send in. If you are attending the CC Volleyball Camp you will also be required to submit the following forms (Blue links below):
POTASIUM IODINE FORM
PHYSICAL FORM: Physical must have been performed within 3 years of the last day of camp. We will keep all physical forms on file until their expiration. If you attended camp last year, we may still have a valid physical on file for you.
SELF-MEDICATION FORM: ONLY NEEDED IF YOU ARE BRINGING MEDICATION TO CAMP. We are a self administer camp. If you are bringing any medication with you (including Aleve, Advil, Tylenol...) you must have an MD complete the following form for each medication you are bringing with you to camp. We must have this form on file for each medication you bring to camp in order to be in compliance with state regulations.
